Two forms, both downloadable below. Filling them in at home is the difference between sitting in the waiting room with a clipboard and going straight through to the chair.
The two forms
Patient Registration Form
Your name, contact details, insurance and who to reach in an emergency. This is the form the front desk would otherwise hand you on a clipboard.
Medical History Form
Current medications, past treatment and anything Dr. Amini should know about before she looks in your mouth.
